NATION’S LAB WARNING, WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK-THIS AGREEMENT MUST BE SIGNED BY ALL PERSONS WHO WISH TO PARTICIPATE IN ANY NATION’S LAB CLASS, EVENT, OR ACTIVITY. 

In consideration of being allowed to participate in any way in the classes, events, and/or activities of NATION’S LAB, I,

Recognize and understand that martial arts training is an activity that involves physical contact and that my participation might result in serious injury, including permanent disability or even death, and severe social and economic loss.

Recognize and understand that such risk may be due to not only my own actions, but also the action, inaction or negligence of others, the regulations of participation, or the conditions of the premises, or of any of the equipment used.

Recognize that there may be other risks that are not known to me or to others or not reasonably foreseeable at this time.

Agree to inspect the facilities, equipment and pairings prior to participation. I will immediately inform an instructor if I believe that anything is unsafe or beyond my capability and refuse to participate.

Assume all of the foregoing risks and accept personal responsibility for any damages that may result from injury, permanent disability or death.

Enter martial arts training and/or competition entirely of my own free will and understand the importance of following the rules of training and competition.

I certify that I am in good physical condition, and have no disease, injury or other condition that would impair my performance or physical and mental well-being during intense training practice and/or competition.

Grant permission in case of injury to have a doctor, nurse, athletic training or other emergency medical personnel provide me with medical assistance or treatment for such injury.

Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and

I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Nation’s Lab, it’s affiliated organizations and governing bodies, their officers, instructors and personnel, other

members of the organizations, participants, supervisors, coaches, sponsoring organizations or their agents, and if applicable, owners and leasers of the premises from any and all liability to the undersigned, his or her heirs and next of kin for any and all claims, demands, losses and damages which may be sustained and suffered on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasees or Otherwise.

PHOTO/VIDEO RELEASE: I hereby grant Nations Lab permission to use my likeness and my property in photograph (Still photo, Video, and Audio) in any and all of their publications, including but not limited to all of Nations Lab’s printed and digital publications. I understand and agree that any photograph using my likeness will be the property of Nations Lab and will not be returned. I acknowledge that since my participation with Nations Lab is voluntary, I will receive no financial compensation.I hereby irrevocably authorize Nations Lab to edit, alter, exhibit, publish or distribute this content for purposes of publicizing Nations Lab’s programs or for any other related, lawful purpose. In addition, I waive the right to inspect or pre-approve the finished products including written or digital copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising out of, or related to, the use of the content. I hereby hold harmless and release and forever discharge Nations Lab from all claims, demands, and causes of action which I, my heirs, representatives, executives, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)

This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward

understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.
